Start a root cause analysis project. You may also use this template to plan and prepare a design of experiments for process or system optimization.

This Template Provides the Following Worksheets:

    SIPOC: use to determine process scope.

    Boundary Diagram: use to determine system scope.
  3. 3
    CTQ Tree: use to determine and prioritize key output variables.
  4. 4
    Ishikawa Analysis: mind map used to determine potential input variables.
  5. 5
    List of Hypotheses: use to keep track of hypothesis to be tested, as well as results obtained.
  6. 6
    List of Analyses / Studies: used to keep track of studies conducted to accept or reject a hypothesis.

    Cause Effect Matrix: use to correlate key input variables and key output variables, Identifying high priority factors to be prioritized and studied. You may skip the use of the cause effect matrix if the root cause has been determined by hypothesis testing.

    Zwicky Box: use to list observations and possible explanations for each observation. Narrow down on the root cause by eliminating mutually exclusive explanations. You may skip the use of the Zwicky Box if the root cause has been determined by either hypothesis testing or design of experiments.

What to Add or Combine:

  • Causal Loop Analysis: Add causal loop analysis for detailed understanding of cause-effect relationships between system components.
  • Fault Tree Analysis: formal fault tree analysis with probabilities. Use to document fault networks and determine probabilities of events.
